Attendee Acknowledgement

The American Chemical Society (ACS) will follow all Center of Disease Control and Prevention (CDC) guidelines and any state governmental requirements and guidelines, as applicable, to reduce the risks of exposure to COVID-19.

Attending the Event: Individuals who fall within any of the categories below should not attend. By attending the ACS Theater Event, you certify that you do not fall into any of the following categories:

  • Individuals who currently or within the past ten (10) days have experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath among others.
  • Individuals who have traveled at any point in the past fourteen (14) days from any region or area that has experienced or is experiencing sustained community spread of COVID-19.
  • Individuals who believe that they may have been exposed to a confirmed or suspected case of COVID-19 or have been diagnosed with COVID-19 and are not yet cleared as non-contagious by state or local public health authorities or the health care team responsible for their treatment.
Event Safety: To promote the safety of everyone at the event, ACS will follow the guidance and requirements of the CDC, California Department of Public Health, San Francisco Department of Public Health (DPH), The Moscone Center (San Francisco Convention Center) and other local regulations regarding COVID safety practices. This Attendee Safety Plan may change from time-to-time until the start of and during ACS Fall 2023.

Photo Release and Consent: I give my permission for me to be photographed and/or videotaped by the media and representatives of the American Chemical Society in conjunction with the ACS Theater Event. I hereby transfer to the American Chemical Society all copyright and other interests in photographs and/or video taken. I also hereby grant royalty-free permission, including nonexclusive world rights in all languages, to reproduce in all formats including but not limited to print and online, to reproduce and include my likeness(es) for promotional purposes of ACS outreach programs.

Disclosures: Your registration and participation in the ACS Theater Event, requires that you read (in its entirety) and agree to abide by the following policies: Attendee Safety Plan - American Chemical Society (acs.org)

Attendee Waiver: As a condition of participation in the Event, I hereby acknowledge and agree for myself, my child(ren), my heirs, and assigns to release, indemnify and hold harmless ACS, its subsidiaries, affiliates, officers, directors, agents, and employees (collectively referred to as “ACS”) from any and all losses, damages, claims, demands, causes of action, judgments or liabilities whatsoever.

I agree that ACS reserves the right to terminate my participation and the participation of any guests for failure to act in accordance with all rules and regulations of the Event or for failure to follow any instructions and directions.
